Current production time is approximately 2 weeks, starting after the fabric choices are finalized.
Use this form to list the order in which you would like your shirts on the quilt. It also has a place to list color choices.
Download Form
Depending on your browser and settings, the form may download immediately or open in your browser. If it opens in the browser, you may not be able to fill it in there (again, depending on your settings). If that is the case, save it from there.
You must either save or print the form in order to email/send it back to me.
Ordering
a Quilt is Easy!
You don't necessarily have to contact me before sending your shirts; but it sometimes helps to know a package is on its way. Plus, we can get a jump on some of the necessary details that help get a quilt finished more quickly.
- Don't cut your shirts! I'll do that.
- If you have already cut your shirts, don't worry; I can still make your quilt.

- If you prefer a street address - to use UPS, FedEx, etc - let me know and I will provide it for you.
- Either send payment with your shirts, or I can invoice you via email. (See Methods of Payment section below)
- Full payment is required upon ordering. That way, as soon as I am finished, I will ship your quilt.
- Make sure I have a way to contact you! A valid email address is required to receive an invoice.
Contact you for color/fabric choices and any additional information
Select fabric choices and send pictures or samples
Make a custom, quality quilt once fabrics have been chosen
Send updates on progress at significant intervals
Ship it to you with tracking and full insurance as soon as it is finished
METHODS OF PAYMENT:
- Checks and money orders - just send it with your shirts.
- Payment via PayPal Account.
- Credit/Debit cards - these are processed through PayPal.
- You do not have to have a PayPal account to securely use a card through the PayPal site.
PayPal is a secure and trusted method for paying for items online.
- Request an invoice, and provide a valid email address.
- You will receive an invoice via email. It will be from Event Quilts.
- In the email, click on the blue "View and Pay Invoice" button.
- A secure PayPal page will open with the invoice details.
- Click the blue "Pay $xx.xx" button in the upper left corner.
- The secure PayPal front page will open.
- Log into your PayPal account to complete payment.
To Pay Using a Credit/Debit Card - if you wish to pay using a debit/credit card but don't have or don't want to use a PayPal account:
- In the email, click on the blue "View and Pay Invoice" button.
- A secure PayPal page will open with the invoice details.
- Scroll down the PayPal page a bit and click the gray "Pay With Debit or Credit Card" button.
- A secure "Guest Checkout" page will open.
- Enter card information to complete payment.
